It is helpful to arrive 15 to 20 minutes prior to your initial visit, and five minutes prior to any follow-up visits. Below is our various forms that are to be filled out by a new patient to Body One along with commonly asked questions. Click on the link below to view, complete and print out the forms. Bring the completed forms along with your insurance card(s), personal ID and therapy prescription from your physician (if your physician has not sent us your therapy prescription directly) to your first visit . Please note: if the patient is considered to be a minor (18 years of age and younger), a minor patient treatment consent form will need to be filled out by a guardian and brought by the patient to the first visit.

We file claims for many insurance carriers and are considered "in network" with the carriers listed below.

What happens if I am "out of network?"
We offer "out of network" patients two options. Some insurance companies offer "out of network" benefits that may partially cover your care. Another option is to pay for services at the time of delivery and receive a 30% discount from our standard fees.
What if I don't have insurance, or I want to pay for services and file insurance on my own?
You may pay at the time of service and receive a 30% discount from our standard fees.
What methods of payment do you accept? Cash, check, credit card (Visa, MC, Discover) and/or Health Savings Account cards.
Can Body One Physical Therapy file claims with a motor vehicle accident/auto or liability carrier? No, you must pay for your services as most personal and comprehensive liability carriers make their payment directly to the patient and not to us as the provider of the services.
What should I bring to my first physical therapy visit at Body One?
You will need to bring your prescription for physical therapy, comfortable exercise clothing, insurance information, completed patient forms and photo identification.
What should I wear for my physical therapy appointment? Dressing as you would on a trip to the gym is appropriate. You will be asked to perform tests and movements that will assist the therapist in establishing an appropriate plan of care, so comfort and flexibility in clothing is important. Patients with shoulder, neck and spine problems should bring a t-shirt or sports bra; patients with hip, knee or foot/ankle problems should bring shorts.
What happens if I miss or have to cancel an appointment? Please help us to provide you with the best service possible by keeping scheduled appointments. Unless cancelled at least 24 hours in advance, our policy is to charge for the second and successive missed appointments at the rate of $40 per missed visit.
